How to use

Testosterone Dual Action is self-injected intramuscularly once weekly, or as prescribed by your provider. Injections are typically administered into the gluteal muscle or thigh. What is Testosterone Dual Action?

Testosterone Dual Action is a compounded intramuscular injection that combines two commonly prescribed testosterone esters: cypionate and enanthate. This combination is designed to offer the immediate benefits of enanthate with the steady, sustained hormone levels supported by cypionate.

How it works

The testosterone esters are absorbed at different rates. Enanthate begins acting relatively quickly—often within a few days—while cypionate provides a more gradual, sustained release over time. Together, they mimic the body’s natural testosterone rhythm more effectively than a single ester alone.

What to expect

Most men start to notice improvements in energy, mood, and sex drive within as little as 1–2 weeks. Physical changes like improved strength and lean mass can take a bit longer, typically 4–6 weeks or more. Full results often build gradually over a few months with consistent use.

Side effects

Some users may experience oily skin, water retention, or mood fluctuations, often during the early adjustment phase. Some men may experience increased estrogen and associated symptoms, though your doctor can prescribe an add-on called anastrozole to help mediate this. Regular follow-ups are included with your subscription to monitor labs and check in on how you’re feeling.

Providing bloodwork

To start this treatment, at your 3-month follow-up, and recurring yearly, you must provide lab results. This helps the doctor ensure that the treatment is working as it should and offers the chance to make any adjustments.

You can use previous bloodwork if they meet the minimum requirements, or obtain new results at your chosen lab. We recommend LabCorp On Demand or Quest Diagnostics.

You'll be asked to submit your bloodwork during your intake, but if you don’t have them yet, you’ll have 10 days to upload them in your patient portal. If we don’t receive them within that window, your prescription request will be cancelled. You’ll also use the portal to upload future results for follow-up visits.

Common questions about Testosterone Dual Action

How do I know if I have low testosterone?

Symptoms of low testosterone (aka “Low T”) can include fatigue, low libido, weight gain, mood changes, trouble focusing, and reduced muscle mass. The only way to know for sure is through a blood test and doctor evaluation.

Who is a good candidate for this treatment?

We generally recommend Dual Action for men who are currently taking another form of TRT and are looking for a step up to a more balance and long-lasting option. Men experiencing symptoms of low testosterone—such as low energy, reduced libido, difficulty building muscle, or mood swings—may benefit. As with all testosterone therapies, lab-confirmed diagnosis of low T is required before starting treatment.

What makes this different from cypionate or enanthate alone?

This dual-ester formula gives you the best of both worlds: the fast onset of enanthate and the long-acting steadiness of cypionate. It’s especially helpful for men who feel fluctuations on single-ester therapy.

What is an add-on and why might I need it?

As testosterone levels increase, it can be converted into estrogen, potentially leading to unwanted side effects like mood swings, gynecomastia, and water retention. To help address this, your doctor may prescribe an add-on called anastrozole, which mediates the body’s estrogen levels, allowing for balanced hormone levels throughout treatment. If prescribed, your add-on and any necessary instructions for use would be included with your prescription at no additional cost.

How do follow-ups work?

If prescribed, you will have virtual follow-ups every 3 months. Follow-ups involve uploading new bloodwork results followed by a few quick questions to help the doctor evaluate your progress and make any changes to your dosage. You will receive text and email communications when it is time to complete your follow-ups, and it is vital that you complete them in a timely manner to ensure the safety and effectiveness of your treatment.

What is the process for submitting bloodwork?

Bloodwork must be obtained from a reputable lab. We recommend using LabCorp On Demand or Quest Diagnostics for convenience. You will submit these labs during your initial visit, or if you don’t have them yet, you can upload them to your profile within 10 days of submitting your prescription request. You will also be asked to submit update bloodwork results along with each 3-month follow-up to ensure treatment remains safe and effective.
